The programs listed here are to help New Yorkers who are too sick to shop or cook for themselves or can't afford to purchase the food needed for medical reasons.
Program Summary
This program provides free medically tailored meals for New Yorkers living with chronic and/or life-altering illnesses like cancer, Alzheimer’s, renal failure, COPD, HIV/AIDS, diabetes with severe complications, or another serious medical condition. The program provides Registered Dietitian Nutritionists to work with clients to design individualized meal plans based on medical diagnosis and nutrition assessment. Our home-delivered medically tailored meal program includes ongoing nutrition education and counseling.

Program Requirements
Completion of an online eligibility screener survey
Registration
Confirmation of a medical diagnosis and the limitation of activities of daily living (ADL) by a medical practitioner.
Program Restrictions
Residents of New York City Only and Hudson, New Jersey.
Payment & Insurance
Services are provided regardless of a person's ability to pay.
