Homeless Outreach Teams
The City of New York provides outreach and assistance to those who are experiencing street homelessness or are not connected with shelter services.
To request assistance for yourself someone else, please contact call 311.
Assistance will be rapidly dispatched to the person in need of assistance, in most cases within an hour.

Case Management
Assistance with completing the HRA 2010E Application.
Assistance with the CityFEPS Application.
Medical and Psychiatric Outreach Teams are available.
Provides connection to transitional housing and safe haven shelters.
Program Requirements
Chronically Street Homelessness, which is defined as at least nine months of street homelessness in the last two years.
Program Restrictions
Single Adults ages 18 years or older.
